(A) Please remember to bring the following to your appointment:

• Insurance ID card (Up-to-date insurance information).
• Driving license or Military ID
• List of all the medications
• Reports of relevant lab test, chest x-rays, CT scans, pulmonary function tests or sleep studies.
• CD's of all the Chest X rays, CT scan and PET scans with reports
• Per insurance regulations: All patients are required to pay the Co-pay, deductible and Co-Insurance.

(B) New Patients

• Call our office to get you access to the patient portal or download the Patient Forms.
• Click the Patient Portal and complete the intake forms online.
• Log in and scroll on to the left column.

(a) Go to "My Account" on to the left column

• Click "Personal Information" and " Additional Information".
• Enter demographic data: date of birth,address, phone number, Insurance information, pharmacy etc.

(b) Go to "Questionnaire" and click "Medical History" on the left column

• All patients complete " Past Medical History"
• Complete the "Sleep Medicine Questionnaire" if you have a sleep issues.
• Complete the "Pulmonary Medicine Questionnaire" if you have respiratory symptoms.

(C) Established Patients

• “Bill Pay” securely via the Patient Portal
• Check and update information, change / cancel appointments
• Send and obtain e messages via the patient portal
• Receive appointment reminders via e mail
